The revised Voluntary Self-Identification of Disability Form (CC-305) is available to download from the Office of Federal Contractor Compliance Programs (OFCCP) website. The revised form updates the preferred language for disabilities and includes additional example of disabilities. Federal contractors and subcontractors have until July 25, 2023, to incorporate the new form into their applicant and employee systems and processes.
